Integrated Clinical & Preventive Care
Omni Group provides accessible primary and preventive healthcare for individuals who face challenges connecting with traditional medical services. Our clinical team supports chronic disease management, preventive screenings, vaccinations, medication oversight, and linkage to infectious disease testing for HIV and Hepatitis B/C. We focus on early intervention, routine follow-up, and coordinated care that keeps clients stable and engaged in their long-term health.
All Hearts Cardiology Remote Monitoring Program
Omni partners with All Hearts Cardiology to offer Remote Patient Monitoring (RPM) for clients living with chronic heart and metabolic conditions. Through connected devices—blood pressure cuffs, weight scales, and cardiac monitors—clients receive daily medical tracking without needing frequent clinic visits.
All Hearts’ cardiology team reviews real-time readings, alerts our staff to concerning changes, and supports early intervention to prevent hospitalizations. Omni’s CHWs assist with device setup, education, and follow-up, ensuring clients understand how to use the equipment and stay engaged in their care.
This remote cardiology collaboration strengthens our chronic care model by providing continuous oversight to individuals who face transportation barriers, unstable housing, or difficulty accessing specialty care.
Behavioral Health, Recovery & OBOT/MAT

Our behavioral health programs address mental health challenges, substance use disorders, and co-occurring conditions. Services include therapy, counseling, psychiatric coordination, crisis support, and recovery groups. Omni operates a Medication-Assisted Treatment/OBOT program using evidence-based medications and ongoing support to help individuals stabilize. We also provide long-acting injectables (LAIs) for mental health treatment and structured recovery education to guide clients toward long-term wellness.

Harm Reduction & Community Outreach
Omni’s harm reduction work meets people where they are—whether on the streets, in shelters, or in local hospitals. Our outreach team distributes essential supplies such as naloxone, wound-care kits, hygiene items, food, and safer-use materials. We provide overdose prevention training, safety planning, and direct referral to treatment and medical care. Our Health & Wellness Lounge offers a safe space for rest, support, classes, and connection to community resources.
Community Health Worker (CHW) & Care Coordination Programs

Community Health Workers help clients overcome barriers related to housing, transportation, insurance, food access, and healthcare engagement. They provide hands-on navigation, appointment support, home visits, medication assistance, and long-term follow-up. Omni also offers Targeted Case Management (TCM), Principle Illness Navigation (PIN), and Community Health Integration (CHI) services—ensuring clients receive structured, reimbursable care coordination through Medicaid and Medicare.

Education, Prevention & Life Skills Programs
Omni delivers classes and workshops that promote health, prevention, and personal growth. Programs include sexual health education, HIV/STI and hepatitis prevention, chronic disease education, nutrition and cooking skills, mental health awareness, financial literacy, employment readiness, and recovery-focused life skills. These sessions are tailored to meet community needs and often held in our Health & Wellness Lounge to increase accessibility.
